Background : Ultrasonography is the diagnostic method of choice for visualization and rational work-up of abdominal organs. The dilatation of the common bile duct helps distinguish obstructive from non-obstructive causes of jaundice. Availability of normal measurements of the common bile duct is therefore important. There exists significant variations in the anthropometric features of various populations, regions and races. Aim : Study was conducted to obtain data on sonographically measured diameters of common bile duct in a series of normal Rajasthani population and to measure its correlation with age, sex and anthropometry.

What is the upper limit of normal for the common bile duct on ultrasound: How much do you want it to be? DOI: Source PubMed. Conventionally, the upper limit of normal for the common bile duct as measured by ultrasound is considered to be 6 mm. This review is a somewhat personalized account of how that number became the convention and cautions the reader to avoid being slavish in the use of this number. Two specific cautions are not to apply this limit to older patients and to consider where in the common bile duct the measurement was taken, as measurements taken closer to the pancreas may be larger than ones closer to the liver.

No full-text available. A dilated common bile duct CBD suggests obstructive causes, which may require invasive imaging or remedial procedures [1]. However, an accurate reference range for CBD size remains debatable [1][2][3] [4] [5][6][7][8][9]. Thus, to determine whether a spontaneous abnormality or atypical dilation is important, there needs to be a reference range such that CBD diameters exceeding the upper limit can be classified as abnormal. With the widespread use of cross-sectional imaging and improvements in cross-sectional imaging technology, the diameter of the CBD is being detected incidentally with increasing frequency when using ultrasound, computed tomography CT and magnetic resonance cholangiopancreatography MRCP [1][2][3] [4] [5][6][7][8][9].

The diameter of the CBD changes in response to various factors, including age [1][2][3] , cholecystectomy [2,3] , measurement location [4] , respiration [5] , and body mass index BMI, which was calculated as weight in kilograms divided by the square of height in meters [3].

For some of these factors, such as age and gender, the effect on the CBD is not clear. Common bile duct diameter in an asymptomatic population: A magnetic resonance imaging study. Full-text available. Jan Aim: To measure the common bile duct CBD diameter by magnetic resonance cholangiopancreatography MRCP in a large asymptomatic population and analyze its some affecting factors.

The age, sex, respiration, body length, body weight, body mass index BMI , portal vein diameter PVD , length of the extrahepatic duct and CBD, cystic junction radial orientation and location were recorded. The subjects were divided into 7 groups according to age. Results: Among the subjects, the CBD diameter was 4. The CBD diameter is correlated with age. Respiration did not affect the non-dilated CBD diameter. About cm after leaving the liver, the cystic duct joins the common hepatic duct to form the CBD.

The length of the CBD varies between 5 and 15 cm, depending on where the common hepatic duct and cystic duct are merged, and the normal width is mm Niederau et al. The CBD is divided into supraduodenal, retroduodenal, pancreatic, and intraduodenal segments. Aging, overweight and previous cholecystectomy are associated with the dilatation Laks , Daradkeh et al.

Jukka Karvonen. Correlation between CBD diameter in sonography and age has been specified in previous studies by Niederau et al. However, in the study of Chuah et al. A Sonographic Study. May In the absence of hepatobiliary symptoms, opioid consumption has been shown to cause dilatation of the common bile duct CBD.

The main objective of this study was to measure with sonography CBD diameters in opioid addicts as compared with nonaddicts. The research was done on individuals; were opioid addicts using various routes of administration inhalation, oral, or intravenous , and had no history of addiction control group.

All patients underwent abdominopelvic sonography, and the internal diameters of the proximal part of the CBD were recorded. The average CBD diameter in the control group was 4. A significant increase in the average diameter of CBD in the case group with the oral route was The diameter of CBD was age dependent. The dilatation of the CBD detected by sonography occurring in opioid addicts in all age groups was attributed principally to the effect of opioids.

Oral administration of opioid caused the highest dilatation effect on CBD. Kaude in his Ultrasonographic study reported a mean average diameter of 3.

Bowie suggested that 9 the upper limit of normal CBD diameter is 7 mm. Our values are little higher than that reported by Admassie in Ethiopian 10 11 population and Horrow et al and Bachar et al 12 in American population. Ultrasonic Measurement of Common Bile Jul Although imaging techniques have been developed significantly in recent years, but there are still many different opinions the scientific community on the effect of age, weight, sex, and Cholecystectomy on common bile duct diameter [9,10].

Several studies have shown that the average normal common bile duct diameter is 4 mm at age 40, and 5 mm at age 50 and 8 mm at age 80 [4,9, 11, 12]. In contrast to these results, Harrow and colleagues found no relationship between age and diameter of the common bile duct [5]. One of the most important factors in bile duct diameter attention in recent decades has been the issue of the impact of drug addiction on bile duct dilatation [13].

So that the study of Poralla and colleagues shows the higher duct diameter in men but there was no significant relation between these two variables in the studies of Adibi and Joshi [13,16,20] Considering the fact the men have a higher body mass index than women and the duct diameter increases with the increased BMI, thus the relationship observed in this regard can be justified [21] The mean diameter of the common bile duct in this study significantly increased with increasing age.

These results have been confirmed by other studies [9, 11, 16,19]. However, the relationship between age and the diameter of the common bile duct has not been observed in the study of horrrow and colleagues [5]. Introduction: Given the importance of identifying common bile duct diameter in diseases diagnoses and being different in different races, this paper aimed to investigate the relationship between the diameter of the common bile duct and demographic factors and drug addiction in patients who referred to the imaging department of Imam Reza hos-pital in Birjand during the year Methods: This cross-sectional-analytical study was conducted on pa-tients referred to the Imam Reza Hospital, Birjand University of Med-ical Sciences, during the year The study population consisted of patients with complaints other than the complaints associated with disorders of the liver and bile duct.

Information about sex, drug use, BMI, age and duct diameter in proximal, middle and distal parts were recorded by the checklist. Data were analyzed using the SPSS software version Results: A total of subjects participated in the study.

The results showed a sig-nificant relationship between bile duct diameter with age, gender, body mass index and addiction. Discussion: According to these results, in the case of dilated common bile duct in sonographic imaging and considering the age, gender, body mass index status and the addiction; findings need to be adapted to his clinical status.

There are controversies regarding the upper normal diameter of the common bile duct CBD but it is conventionally accepted to be 7 mm [2][3][4][5] [6]. A variety of factors can influence bile duct size, prominently imag ing modality, age [7][8][9][10] [11]. Several studies in the last 20 years reported an increase in the CBD diameter in older patients, even if with consistent variability [6, 7,9,10,12]. Based on autoptic observations, some authors identified loss of elastic fibers and proximal compensatory dilatation due to distal sclerosis as potential causes of the phenomenon [13].

Endoscopic ultrasound in common bile duct dilatation with normal liver enzymes. In recent years, the description of isolated bile duct dilatation has been increasingly observed in subjects with normal liver function tests and nonspecific abdominal symptoms, probably due to the widespread use of high-resolution imaging techniques. However, there is scant literature about the evolution of this condition and the impact of endoscopic ultrasound EUS in the diagnostic work up. When noninvasive imaging tests transabdominal ultrasound, computed tomography or magnetic resonance cholangiopancreatography fail to identify the cause of dilatation and clinical or biochemical alarm signs are absent, the probability of having biliary disease is considered low.

In this setting, using EUS, the presence of pathologic findings choledocholithiasis, strictures, chronic pancreatitis, ampullary or pancreatic tumors, cholangiocarcinoma , not always with a benign course, has been observed. Data point out to a promising role of EUS in the identification of a potential biliary pathology.

EUS is a low invasive technique, with high accuracy, that could play a double cost-effective role: identifying pathologic conditions with dismal prognosis, in asymptomatic patients with negative prior imaging tests, and excluding pathologic conditions and further follow-up in healthy subjects.

The purpose of this study therefore, is to determine the normal CBD diameter in healthy subjects or subjects presenting to hospital with medical conditions other than those of the hepatobiliary system in a defined Nigerian population. Mohammad H Mohammad H. Therefore, computed tomography CT , which has been used to measure the CBD diameter in adults or post-cholecystectomy, 2 and magnetic resonance cholangiopancreatography MRCP are applied to show the whole CBD structure, to find the portion with the largest diameter and to detail any abnormalities of the CBD.

Initial large diameter of common bile duct is associated with long-term dilatation of bile duct after endoscopic extraction of stones. Sep To investigate the time and extent of recovery of dilated common bile duct CBD after extraction of CBD stones and to identify factors related to long-term dilatation of the CBD. A total of consecutive patients undergoing endoscopic extraction of CBD stones between January and December were recruited.

After exclusion, 44 patients were analyzed retrospectively. Computed tomography or magnetic resonance cholangiopancreatography was used to measure CBD diameter. However, the diameter in patients who were followed up for more than one week did not differ compared to that in patients who were followed up for less than one week. B ductal dilatation but is not sensitive for detecting choledocholithiasis, partly because of difficulties in visualizing the distal common bile duct [11].

The normal diameter of the common bile duct is variable, but a diameter of greater than 7 mm is generally indicative of bile duct obstruction among patients without previous cholecystectomy [12]. Although CT is not the best technique for imaging choledocholithiasis, it is frequently performed for the assessment of jaundice.

The size of the common bile duct is a predictor of biliary obstruction and it's measurement is therefore an important component in the evaluation of. The CBD is approximately 8 cm long and usually <6 mm wide in internal diameter but this can be dependent on a number of factors including age and prior.

The common bile duct , sometimes abbreviated CBD , [2] is a duct in the gastrointestinal tract of organisms that have a gallbladder. It is formed by the union of the common hepatic duct and the cystic duct from the gallbladder. It is later joined by the pancreatic duct to form the ampulla of Vater. There, the two ducts are surrounded by the muscular sphincter of Oddi. When the sphincter of Oddi is closed, newly synthesized bile from the liver is forced into storage in the gallbladder.

The biliary tree descends from the canaliculi at the hepatocytes, gradually enlarging and merging to the right and left hepatic ducts. A smaller accessory pancreatic duct and sphincter is usually present but rarely visible from the dorsal pancreatic embyologic rotation.

Accepted: Abstract Traditionally, the common bile duct CBD has been said to measure up to 6 mm in patients with gallbladders and up to 8 mm in cholecystectomized patients. However, these recommendations are based on very old studies performed with trans-abdominal ultrasound.

On ultrasound imaging, it is not always possible to confidently see where the cystic duct enters the common hepatic duct to form the common bile duct. Therefore it is common practice to use the term common duct CD as a term conflating the common hepatic and common bile ducts. It joins the pancreatic duct at the ampulla of Vater , which drains into the second part of the duodenum through the major duodenal papilla. There are four main relationships of the CBD with the pancreatic head 2 :. Anatomy: Abdominopelvic. Please Note: You can also scroll through stacks with your mouse wheel or the keyboard arrow keys.
